Cause 1: rankings are being managed, not leads

This is the most common one. Many agencies proudly report on positions and visitors. Nice, but that is not what pays your bills.

You can rank number 1 for a term that not a single customer uses. Lots of traffic, zero leads. A good approach starts with your business and your leads, not with vanity rankings.

Many sites target broad, informational terms because they have high volume. But volume is not revenue. The right terms are often searched less and sit closer to the purchase. That is where your leads are.

Sometimes the SEO works perfectly and the traffic arrives, but nothing happens. In that case the leak is not in your visibility, but in your website.
